Academic Documents
You can order official transcripts, certificates, proof of enrolment and results notices from your studies at Otago Polytechnic.
For privacy reasons, you can only request your own records. You will be asked to confirm that you are the individual named in the request as part of the form.
Currently enrolled or incomplete programme
If you require documentation for a programme you are currently enrolled in, we can issue you a Certified Results Notice. Please select Certified Results Notice.
You can order a transcript once you have graduated or if you have withdrawn.
NZQA
If your qualification title begins with “National Certificate in…” or “National Diploma in…” then your certificate is issued by NZQA, not Otago Polytechnic.
Please do not complete this form. Instead, you’ll need to request a reprint directly from NZQA using their online form: NZQA Certificate Reprint Request

Processing time
Requests are typically processed within 10 working days from the receipt of payment. Additional time may be required if your records are from before 2013.
Damaged or incorrect academic documents
If you receive your documents and they have been damaged in transit or have a printing error, please get in touch and we will fix this for you, free of charge. This includes details like missing information, incorrect results, spelling errors or marks that shouldn’t be on the page. Please note that damage that occurs after the documents are in your possession is not covered, and will require you to request and pay for a new document.

International courier
International courier will incur a $50 fee for any address outside of New Zealand.
Study completed before 2013
If your study was completed before 2013, an additional payment of $25 is required for us to search your records.
